This comprehensive plan is designed to provide families and individuals with a wide range of preventative affordable dental services and discounts for additional services for one yearly price. The dental discount plan is available in Blue Ash, Batavia and Lebanon dental offices and is good for use at all our locations.
For those without dental insurance, our dental discount plan can save families more than $300 per year on routine cleanings, x-rays, and checkups.

Terms and Conditions
- All family members must live in the same household.
- This is a dental discount plan and not dental insurance and cannot be combined with any other insurance plan.
- Membership dues are non-refundable and must be paid in full before enrollment begins.
- If payment is required, it is due in full at the time of service.
- Yearly membership fees cannot be financed with CareCredit.
- The effective date of the plan is the day you enroll and is good for 12 consecutive months.
- Cleanings must be completed before the end of the coverage date.
- Membership is limited to Moorehead Dentistry, if you must be referred to a specialist, discounts will not apply at that location.
- The 25% discount does not apply to electric toothbrushes, prescription toothpastes/rinses, fluoride, whitening gels, or take-home products.
- Discount does not apply to full arch implant supported restorations or orthodontics: including Invisalign, SmileMoore, orthodontic appliances, and other orthodontic services.
- Membership fees or percentage discount is subject to change on a yearly basis.
- Procedure fees are subject to change at office’s discretion.
